So you need campaign or battle advice?
I can give some advice on your army's composition: Polish Nobles!!!
Polish Nobles should be your main cavalry unit. Their Javelin attack should be used to their max. Their stats are quite average for late game usage but their javelins even it all out.
Strzelcy should be your main support cavalry. Since they have crossbows they are usefull throughout the entire game.
Your infantry is a totally different case. I usually use a mixture of Spearmen, Dismounted Polish Nobles (or higher) and Woodsmen. Poland really lacks on their anti cavalry invantry so you have to make certain considerations. Woodsmen are cheap but cannot handle direct cavalry charges and Dismounted Polish Nobles dont last very long either.
In archers you have a choice. Lithuanian Archers or Dismounted Lithuanian Cavalry. I prefer Lithuanian Archers since they can deploy wooden stakes at the start of the battle. But if I recall correctly the Dismounted Archers are better in base missle attack.
Eventhough it doesnt look like it in the first place, Poland has the best cavalry roster in the game. Hussars are in my opinion the best light cavalry in the game and Polish Knights are in the same league with Chivalric Knights(France) and Polish Guards are in the same league with Noble Knights(France). But in addition to your heavy cavalry you have the game's best Missle Cavalry and you should exploit them.
I have no advice on the campaign because I dont like any of the Kingdoms Campaigns. But I do know that Poland has to expand quickly towards the Teutonic Order and HRE before they expand towards you.
